8:30am - 1:00pm, 2:00pm - 4:00pm

To order a prescription please use one of the following methods or use the online order forms listed below.

NOTE.  Prescriptions can not be collected from  the Waverley Medical Centre. You can only collect from your nominated Chemist.

We aim to complete a prescription request within 48 hours of receipt at Waverley Medical Centre..

CHEMIST 

Hand your prescription into the chemist of your choice who will make the request on your behalf. 

Put your prescriptions into the post boxesin the Waverley Medical Centre Waiting Rooms and we will process it for you and forward to a chemist of your choice.

POST

Post it to us and we will process it for you and forward to a chemist of your choice. 

Please re-order your prescriptions in plenty of time.

If you require an item thats not on your reorder slip, you can write it on the bottom or on a separate slip of paper if required. 

Requests MUST state the medication by name or it can not be completed for you.

Our computer system will print the correct medical name for your medication, this may differ from the name which you are familiar with. If in doubt ask the pharmacist or telephone the medical centre for advice.
