Your Personal Health Information  To enable the Practice to ensure that patients receive appropriate health care and treatment, a large amount of patient-identifiable information is collected. As well as enabling relevant Practice staff to share information to function effectively, personal information has to be shared in certain circumstances with other agencies to ensure that on going care of a patient is not compromised. All patient information is governed by the strict Laws of the Data Protection Act and the long-standing traditions of medical confidentiality. The patient's confidentiality is never compromised.
